The codes and arbitrariness of the system are designed not to deliver better care but to ensure higher profits for insurance companies. Delay and denial are a feature, as they say, not a bug.

I don’t see any way out of basing revenue on a risk model, and any risk model is going to run off codes.

If we got rid of the codes people would complain about poaching healthier people and denying care.
